Job Description: As a Home Health RN, you will play a crucial role in providing compassionate and effective care to our patients. You will have the opportunity to work closely with patients in their own environment, helping them maintain their independence and improve their quality of life.

Key Responsibilities:

• Conduct patient assessments, including physical, emotional, and social health.
• Develop and implement personalized care plans in consultation with physicians.
• Provide skilled nursing care, such as wound care, IV therapy, administering medications, and catheter care.
• Educate patients and their families on managing illnesses and injuries.
• Monitor patient progress and make necessary adjustments to the care plan.
• Coordinate with a team of healthcare professionals to ensure comprehensive care.
• Maintain accurate and detailed patient records.
• Demonstrate empathy, patience, and a professional demeanor in all interactions.

Qualifications:

• Current RN license in the state of New York.
• Minimum of one year of nursing experience, preferably in home health or a related field.
• Strong clinical skills and knowledge of home health care protocols.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Driver’s license and reliable transportation for home visits.
