Chief Care Management Officer

Spring Valley or Brooklyn, NY

$150,000 - $175,000


The Chief Care Management Officer directs the development, revision, and implementation of care management policies and procedures to ensure alignment between processes and NYS/ OPWDD Health Home regulations. Collaborates with quality department to set quality goals and establish methods and tools for staff audits, evaluation, and assessments.


Responsibilities:


  • Spearhead the development and implementation of care management strategies, fostering a patient-centered approach to healthcare delivery.
  • Provide high level oversight to the VPs of Care Management and Care Management workforce ensuring their growth and success.
  • Collaborate closely with key stakeholders to identify opportunities for improvement and implement best practices in care management.
  • Monitor and evaluate the effectiveness of care management programs, identifying areas for growth and improvement.
  • Responsible for ensuring that VPs of CM are providing proper oversight to their Regional Director teams and Care Management staff in their established region.
  • Participate in regular meetings with the VPs of CM and other CM staff as necessary.
  • Establish and maintain relationships with the appropriate OPWDD Regional Offices.
  • Maintain ongoing contact with OPWDD for escalated issues and complaints.
  • Responsible for working closely with the VPs of CM to ensure care management staff are maintaining compliance with regulatory requirements as well as polices, maintaining the highest levels of patient safety and quality care including but not limited to, PTO, overtime, etc.
  • Ensure compliance with all OPWDD regulations and polices.
  • Participation in Family Webinars as appropriate.
  • Participate in mandatory meetings at the Corporate Office as deemed necessary.
  • Represent on various Statewide and CCO committees.
  • Complete other duties as assigned.


Qualifications:


  • Outstanding leadership qualities
  • Proficient in understanding the coordination and management of services for individuals who have an intellectual or developmental disability
  • Ability to negotiate multiple systems through interaction with other departments including but not limited to Not-For-Profit IDD providers, health care system
  • Solid verbal and written communication skills
  • Ability to handle sensitive and confidential information.
  • Organizational skills
  • Bachelor’s degree in Human Services or related field is required.
  • Master’s Degree or licensed professional, preferred.
  • A Minimum of 10 years’ related Human Service experience, of which 5 is IDD.
  • Three years of previous experience at a senior management level.
